NOTE TO PARENTS:  If you turned in the blue Field Trip Medical Information Form on Choir Orientation Night, DONT FORGET TO MAKE A COPY FRONT/BACK OF YOUR MEDICAL INSURANCE CARD AND SEND IT UP TO MR. GABRIELSEN WITH YOU STUDENT.  Thanks!
